I think there's an issue downloading from iCloud drive if you're still running Mavericks on your Mac as this can't access iCloud Drive and I don't think "old" iCloud stuff is moved to Drive until your Mac updates. The second part, you refer to going to Finder so it sounds like a Mac not mobile device issue - is that the case? If so, try removing the icon from the dock then drag it there again from Applications.
